Health Care Spending Essay

In the past seven decades health care spending has been increasing tremendously. The industrial era brought in a new system to help population duel with the cost of medical care. New technology has become a much needed investing in the 21st century which has yield to a great margin between small and big facilities. When looking at all of these factors there is no wonder the nation is spend way too much money on health care, although at times might seem that there is not enough. It seems that the right investment is needed to help aid those that need help the most the elderly and unemployed, with the economy in a fragile state and unemployment rates increasing with the quarters there is no question that decisions about how we are spending on health care needs to change. Even though so many factors are involved in the spending this paper would focus on our current level of expenditures, how and where is the money spent, forecasting what would be coming next. Current level of national healthcare expenditures According to â€Å"Physicians for a National Health Program† (2012), â€Å"National Health Expenditures was $2.8 trillion, and the spending as a percent of GDP was 18% for 2012† (National Health Expenditure Projections: Modest Annual Growth until Coverage Expands and Economic Growth Accelerates). When looking at these facts many would even argue we are spending quite enough when it comes to healthcare or perhaps too much, however is the complete opposite not enough spending is in the right place. These figures does not represent the spent on one area of health care the graph below will provide an idea as to what, when, and where was spent on 2009. As shown on the graph 51% of the spending is towards hospital care and physician/clinical services which are great to be invested in, however the rest of the spending in my opinion is not proportionally distribute, yes those areas are extremely important but it would be beneficial if more can be distributed along medication, and other professionals services to include the testing and diagnose of serious illnesses such as cancer. The investment is require to subsidize the needs on these areas, 45 percent of people under age 65 who don’t have insurance coverage for prescriptions said they had not filled a prescription in the last year because of the cost. Additionally, 84 percent of working-age people in the U.S. without insurance coverage for prescriptions said they had taken some action such as spending less on groceries or postponing paying other bills in order to pay for their medications, an increase from 71 percent last year (Rowan, 2012). Regarding testing for patients with cancer there should be more funding for the new technology available for test that many patients are not aware, because they cannot afford it or simply because it is not available in their area. Spending: Too much or not enough? In 2010, $2.6 trillion was spent on health care services and products, 61 percent of which purchased hospital care, physician and clinical services, and retail prescription drugs. Private health insurance paid for 33 percent, out-of-pocket sources for 12 percent, and other third party payers and programs for 7 percent. The two largest government health care programs, Medicare and Medicaid, purchased $925.1 billion worth of health care goods and services in 2010, accounting for 36 percent of total health care spending (Centers for Medicare and Medicaid Services, 2012). The increase of cost would continue to rise according to economists, health care managers, and advisors forcing for a new reform to change the system. Thus many are not too please with this idea is a phenomenal that cannot be avoid, there is a need to change in policies and methods in which we spent or invest in health care. Suggesting that the investment is too little would be a denial statement, everyday more and more patients are relying on programs such as Medicare and Medicaid to get the attention required at the moment. As the economic recession set in deeper less would be available to afford just the basic needs, medication or any treatment; unemployment rates are on the rise and it does not seem it would stop anytime soon. Although the new reform would try to establish some type of stability, until the insurance companies and policy makers come to an agreement to provide a universal premium for service which can be affordable for all, the same problems would continue to emerge. Many factors also have effect on spending considering for example the demand for new technology in the 21st century; this trend has become to some degree a factor in the rise on health care spending. Much of this new technology is beneficial if it provides a new way to used already establish technology, however many studies have revealed that most of them are only here to replace such technology and at some circumstances increase the cost of service. As a country a deeper consideration of such technology is needed not only to save money, but to invest wisely as to what would help the system cut in spending. Future economic needs of health care For years to come the enduring effects of the economic recession and slow recovery, due mostly because of partial growth in incomes, are expected to continue to limit health spending growth. In 2014 the coverage expansions laid out in the Affordable Care Act for Medicaid and for private health insurance are expected to increase the growth rate for health spending, with notable increases expected in spending on physician services and prescription drugs for newly insured patients. By the end of the year, higher income growth and the continuing shift of baby boomers into Medicare are expected to cause health spending to grow roughly two percentage points faster than overall economic growth, which is about the same differential experienced over the past thirty years. Therefore a necessary reform of a national health care perhaps would provide the change needed, the only issue with this suggestion is the funding. Thus a national health care would help every citizen reach the so needed medical attention required at an affordable price there is no surprise that the funding through Federal programs are thinning, and the cost of private insurance keeps on raising. Therefore a more evaluated reform should be put in place where the cost of care should be regulated, and more affordable options are given to the population; an increase on reimbursement of healthcare for basic health needs is necessary to subsidize the cost for complex health needs in order to keep the cost regulated. According to â€Å"Centers For Medicare And Medicaid Services† (2011), â€Å"In 2014, national health spending is projected to rise to 7.4 percent, or 2.1 percentage-points faster than in the absence of reform, as the major coverage expansions from the Affordable Care Act (ACA) are expected to result in 22 million fewer uninsured people† (National Health Expenditure Projections 2011-2021). Increases in Medicaid spending growth, and private health insurance growth will contribute to the overall acceleration in national health spending in 2014. There is no question that technology would also continue to raise to help the system provide the best and fast way possible services to those in need. Investing more in health care can only benefit the nation; however the right decisions needs to be made in order to keep the spending growth under control. Ksa Self Eval

I am definitely a quick learner which is very beneficial to on the spot coverage for a co-worker, was trained for basic process to handle JABBER for only half a day since my co-worker needs to go to an emergency leave for 3 weeks, midyear n 2013 and helped my peer with relaying referrals to JABBER when she had a foot surgery which leads me to be an alternate on handling JABBER. I was managing authorizations for all local primary care that goes to a native facility, SEC Orthopedic and .NET after ELL was passed on to one of my peers.I was assigned to be a mentor to new employee to assist with all work related issues. On late December 201 3, was asked by my Supervisor to manage JABBER in place of my peer that volunteered for another task. I was handling all officer consults, authorizations and referral management along tit SF which is also considered a big task due to a number of patients that's being sent to their facility and all other local primary care that are being sent to native facilities for almost 9 months. Created a spreadsheet via Microsoft Excel for all JABBER clinics for easy tracking.I volunteer to work TOT every Saturday to take down Mammography consults and all other categories that needs to be worked on. I have been asked questions and advice by my peers (Nurses and Ma's) on work related issues, especially new employees. Questions about work processes, most specially computer related, mostly bout trouble shooting and vendor information. I also helped on answering the phones (ACED Line) when needed. I was the first MASS in our department to have tried the process of the PC program. Wowed some of my peers the step by step process of submitting consult/ referrals/ authorization through scanning to the program. I practically started the process and showed/ trained some of my peers, even the leads. I created the letter for veterans (fixed it, since it was from a flyer in PDF format that needs to be converted to Microsoft Word document), I also created the main coversheet for PC we use whenever we amend, deny, re-route a secondary request/ authorization and put together a verbiage to be used whenever we create an authorization for Tries. Tot tied to work on JABBER, SF and other local primary (native facility) which leads me to just occasionally helping with PEG. However, the main person who has been processing PC authorizations still seek my advice most of the time, as far as trouble shooting PC processes. I also contributed a lot to our temporary MASS working with faxes even though was not assigned to be her mentor, she always seeks my advice most of the time. I am one of the Ma's who were trained to close/ complete consults by scanning medical notes/ documents to CPRM.Although am not the main person for PC anymore, I volunteered on updating and forwarding the PC Provider List every Monday. Currently, have been helping to close out consults (since almost the end of fiscal year) by scanning medical documentations/ notes to CPRM. Added the ASK submitted when I was promoted for GAS 6 just for a refresher. GAS-6 Medical Support Assistant (Advanced) Demonstrated Knowledge, Skills, and Abilities: 1 . Ability to collaborate, communicate, set priorities, and organize the work in order to meet deadlines, ensuring compliance with established processes, policies, and regulations.In my current position as a Medical Support Assistant with the VASS Integrated Care Service, I am responsible with the review (after being approved by MR.), scheduling and authorization process of consults in collaboration with the CICS patient care team and communicating with non-VA medical facilities, this include determining urgency of care to ensure timely and best purchased care for our veterans. I've been constantly picked by my supervisor to process urgent or implicated consults on account of having an excellent attention to details and accuracy.Due to my ability to learn new skills quickly, I became a part of the reconciliation team r egardless of being new, working on a spreadsheet via Microsoft Excel consisting of over 2000 plus claims to determine, track down, adjust or cancel used or unused authorizations on a weekly task to recoup funds that had been obligated to vendors but never used, it was then used for processing backlogged pending authorizations for this FYI. I assisted going through a spreadsheet determining which authorizations needs to be closed (nickered) out freeing up funds for use of unprocessed and pending authorizations. 2. English Class Reflection Paper Essay

It’s hard to believe that my amazing journey in English 104 is coming to an end. The journey has been so much more enjoyable than I initially expected. I have tried to avoid taking English 104 numerous times because reading and writing have not been my strong points. Each time I attempted to take this course, I dropped it due to a particular instructor style, or a boring set of literary materials. This time around was different, as I set my goal to obtain my bachelor degree in Business Administration by the end of this summer. Therefore, I was determined to force my way through this class no matter what challenges I had to endure. To my pleasant surprise, this class was much better than the others. I felt the information and learning come much easier. I enjoyed the class materials and the professor did a very good job responding in earnest to my submissions. Because of the much better experience, I truly believe I have become a better writer and a better critical thinker. Through the discussion board and the professor‘s guidance of our writing assignments, I have improved my analytical skills as well as developed a greater organizational skills. The discussion board was an important tool in developing my understanding of different aspects of the literature reviewed in this course. This new discussion format allowed me to read other students contributions on the same topic, as well as Professor Duclos-Yourdon’s direct and prompts for a deeper analysis. For example, in week five discussion three, we discussed Sister James‘ character, as I said â€Å"Father Flynn offers reasonable explanation about the situation and his involvement with Donald and Sister James’ doubt is appeased. This shows that Sister James is easily persuaded and has no eager to actively seek the truth on her own†(Vickery DB) Following my thread, Kimberly raised a very interesting question by adding, â€Å"who is the real criminal in this play Is it Father Flynn with what reported or is it Sister Aloysius as she manipulates Sister James for her own benefit?†(Voss DB) Through engaging discussion with students on a regular basis, I was able to develop a more detailed understanding of literature work. Additionally, alternative views helped me to incorporate logic and critical thought into my arguments. As a result, I am better equipped to gather necessary information to begin writing my assignments. Professor Duclos-Yourdon’s guidance was crucial in helping me develop my critical thinking skills. Our first literary analysis essay was the most difficult one for me in this course as I had not written an academic paper in nearly ten years. I decided to write my first paper on â€Å"The Yellow Wallpaper† and wanted to submit my rough draft for initial feedback. For fear of not achieving the minimum 1000 word requirement, I began to construct random ideas in my paper, as the professor commented, â€Å"Your argument should stay focused on the story. These larger implications can be raised in the conclusion if they come naturally from your argument.†(Dulcos Essay1) Later, she suggested, â€Å"To develop this argument, you could first write a paragraph establishing the wallpaper as a symbol for entrapment (as you have done). Move from there to the narrator’s loss of individuality, and finally explore the narrator’s transformation into this shadow woman.†(Duclos Essay1) These comments resonated with me and gave me great ideas on the direction I wanted to take the paper. I began to interpret the evidence in a deeper way and rethought my thesis statement to be more focused. Moreover, I deleted the unnecessary paragraphs, reorganized my ideas and supported with factual evidence. As a result, the comments I received from Professor Duclos-Yourdon were so insightful it helped not only develop confidence, but also resulted in a perfect grade. With the additional practice of literary analysis in this course, I conclude that writing is a lot like mathematic. Like mathematics, where there is a specific formula to follow to get a result, so there is also a formula in literary analysis to construct an organized essay. I learn in my previous English class that an essay consists of some very basic elements, for example, an introduction, three body paragraphs, and conclusion. It all sounds really easy, but without following some formula, the result wanders and is difficult to follow. What helped me in this course was learning to break paragraph down into more detail. For example, the topic sentence should clearly tell the reader what the paragraph will explore. Then, simple lead in sentences should be followed by evidence from the literature work. Finally, I complete the prograph with my own interpretation of the evidence. It’s a pretty straight forward method and I can relate to it more as I love formulas. So every time I got baffled, I think of my paper as this giant math equation to solve. Taking these baby steps has helped me in completing all my assignments without feeling defeated. Finally, I realize the importance of taking English classes during the first two years of one’s college career because it prepares a person to communicate well in written words as well as spoken. Although I waited till the final year of college to take this course, the analytical, writing, and organizational skills I learned will benefit me for many years to come. In numerous ways they somewhat become brainwashed by Americans who live their lives not in reality but in idealism, and they attempt to live their life like them as well. However, once they return to their homelands after believing they have obtained their goals, they cannot figure out why their families and friends no longer wish to speak to them. They have altered and become intemperate people resembling us Americans and have brought back terrible habits and ways to their homelands that their people have despised in their customs for several years. Americans are truly paradoxical people who say one thing and do another. We  are constantly contradicting ourselves and others and cannot comprehend why we are so unhappy. We make every effort to achieve happiness but nothing ever seems to be good enough. John Steinbeck expresses that our priorities are extremely wrong in this quote, Americans are remarkably kind and hospitable and open with both guests and strangers; yet they will make a wide circle around the man dying on the pavement. Fortunes are spent getting cats out of trees and dogs out of sewer pipes; but a girl screaming for help in the street draws only slammed doors, closed widows, and silence (Steinbeck 1). Steinbeck puts across a serious point and I boldly agree that we Americans have our priorities twisted around. Ultimately, Americans have a large number of adjustments they will have to carry out before they will ever be able to achieve their American dream and be content. We are so full of dissatisfaction and failure to stick with our goals. We tread on friends and family who get in the way of our so called American dream, and after we believe we have attained it we are not sure as to why we are so miserable. The horizon line for strategic planning and forecasts in most cases typically ranges from three months to five years. Trying to imagine what the world of business will be like 20 years from now is a challenging and noble goal but a researcher is exposed to several risks. One of them is to be too futuristic and end up with a science fiction which would have a little correspondence to the objective reality. The importance of the literature search is to avoid this trap and ground the dissertation on realistic, reliable and reasonable facts and evidences. It will also support the designing of research methodology and developing relevant questionnaire for conducting interviews with IBM and non IBM top management and executives. To build the vision of the future and understand the big picture of the macro-environmental influences PESTLE analysis concept (Figure 2) will be used. A PESTLE analysis is often used as a generic orientation tool, finding out where an organization or product is in the context of what is happening outside that will at some point affect what is happening inside an organization (CIPD, 2010). Figure 1. PESTLE Analysis Political factors Most of the researchers agree that by 2025 there will be a global multi-polar system with rise of China, India and some others. Power will be more dispersed with the newer players bringing new rules of the game while risks will increase that the traditional Western alliances will weaken (Global Trends 2025: Transformed World, 2008). USA will remain the provider of global security (FreeWorldAcademy, 2005) but will have to share the power with other new players. Researches expect China becomes a democratic country in 2030 and gain more political power. Some experts also expect greater Asian regionalism by 2025, which may lead to establishment of new quasi-blocks: North America, Europe and East Asia and new wave of political and economical competition (Global Trends 2025: Transformed World, 2008). The potential for conflict and instability is expected to increase in Middle East and North Africa. At the same time McKinsey Global Institute predicts the acceleration of Africas growth where government actions will manage to end political conflicts and improve business climate (McKinsey Global Institute, 2010). Its also expected that global negotiations between regional blocks and national governments on major issues for the planet, such as biodiversity, natural resources, energy policy and trade regulation will increase and possible result in establishment of new alliances and communities. Driven by higher world population, competition for natural resources, pollution levels Green will become fundamental geo-political and business imperative for the planet. Economic factors GLOBALISATION By 2030 the world will become flatter with an expanded base of financial power where barriers to the flow of goods, capital or knowledge will be almost removed (A View of the World in 2030, ACCA, 2010) which will more regulation on a global level. Its also expected that the global power will shift to emerging markets. This statement confirms Goldman Sachs Group Inc. that the market value of emerging-market stocks may grow more to $80 trillion in two decades, overtaking developed nations, as China becomes the worlds largest stock market (Bloomberg, 2010). Multiple centers of economic power will be distributed around the globe and organizations/countries will need to be culturally adaptable to compete in global markets Figure 2. demonstrates forecasted transition of GDP by 2030. This shift can be explained by several reasons : increases in oil and commodity prices generated big profits for the Gulf states and Russia lower costs combined with government policies have shifted the focus of manufacturing and some service industries to Asia power will shift to the places with political and economic stability (Global Trends 2025, 2008) The world GNI will also undergo the evolution and regional repartition. Compared to the population growth rate 1%, it means that the world income per capita will increase by 2.4% per year over the period Asia becomes a strong visible player in the regional GNI repartition (Figure 4). Estimated $4.3 trillion in annual expenditures in 2008 becomes $32 trillion in 2030 and comprise about 43% of worldwide consumption (2030 GDP Forecast, Future growth of Asian Countries, Cabinet Office, Government of Japan, 2010) Several researches and reports expect the rising BRIC powers which might collectively generate the original G-7s share of global GDP by 2040-2050 (Global Trends 2025, 2008). Meanwhile other sources suggest that new business opportunities will significantly rise in so-called BIICS-Brazil, India, Indonesia, China, and South Africa-shaping up to be business hot spots for many years to come (Working Beyond Borders: Insights from the Global Chief Human Resources Officer Study, IBM, 2010). But the truly new business frontier is named Africa where a group of countries already demonstrated ability to improve macroeconomic conditions, privatised state-owned enterprises, increased openness of trade and lowered corporate taxes, strengthened regulatory and legal systems, provided critical physical and social infrastructure(McKinsey Global Institute, 2010). The rate of return on foreign investment is currently higher than in any other developing region. These factors make this region very attractive for further business development and investments. According to Paul Nunes, Executive Research Fellow at the Accenture Institute for High Performance: Businesses not planning and acting now [to get into Africa] will miss the boat as many did when it came to China. Researchers forecast the standards of living will have risen by 2030. Number of high income people will increase by 8%, number of low income will decrease by 14% but inequalities will remain (Global Trends 2025, 2008). In essence, the economic world will look different than the world today. There will be multiple centers of economic powers, new players will mount the economic and political scene creating new business opportunities, overall, globalized and multi-polar world which will require organizations to re-think their strategies and re-shape their businesses to take full advantage of new opportunities and prosper. Social factors The next 20 years will be characterized by serious and inevitable demographic changes and shifts and their economic, political and social impacts will be enormous. Researchers highlight key demographics marked trends of years to come. World population will increase to 8.2 billion by 2030 with the largest increase projected in Asia and Africa and less than 3% will occur in Europe, USA, Canada, Japan (Global Trends 2025, 2008) Figure.. Population shifts will continue Africa will have the worlds largest working-age population. In 2008 the continent had more than 500 million working-age people and by 2040 its forecasted to exceed 1.1 bln (Figure, 6). Its an open question whether Africa will be able to educate its workforce to make it serious engine for business world. Figure 6. People across the world will be able to access enormous information including digitalized knowledge. Increased capacity to combine and connect know-how, share knowledge, information and best practices spending significantly less time to send and receive information will lead to people being more interconnected, united by new communities and networks (IBM, 2010). More than 5 bln people will be connected to mobile services (The Economist, 2011) and overall speed of live will rise. The other change which is expected to influence the workforce is massive crowd of computers replacing people in a number of jobs. Forcing the economic growth and social transformation technology will continue to reshape consumer needs, requirements, access to information, choice and interactivity models and demand companies to tap the power of technology to improve their competitive advantage (McKinsey and Company, 2010). Legislative factors The expected changes to happen in the world will place a pressure on labor regulation to force their activities in accommodating a networked, open and competitive world. The worldwide focus on growth will require companies to fundamentally rethink how they manage human resources and overcome barriers. Based on IBM 2010 Global CHRO Study, emerging ways of attracting talent like outsourcing, crowdsourcing, use of freelancers, bringing back retired workers which will assume ever greater importance and necessity to mange new forms of employments will require governments not only to develop new labor regulations but also modern information security norms and standards. The highest standards of corporate citizenship will be required by talent, by governments, by the media. There may not be world law in the foreseeable future, but the worlds legal systems will be networked. The Global Legal Information Network (GLIN), a database of local and national laws for more than 50 participating countries, will grow to include more than 100 counties by 2010. The database will lay the groundwork for a more universal understanding of the diversity of laws between nations and will create new opportunities for peace and international partnership. -Joseph N. As describe by OECD (Environmental Outlook to 2030, 2008) there are several key environmental challenges the humanity will have to deal with: climate change, biodiversity and renewable natural resources, water, air quality, waste management. To respond to these challenges governments will have to take the responsibility and proactively start developing a serious of actions to protect the environment. Business and consumers will play a key role to help prevent future environment problems. Its obvious that Green will become fundamental geo-political and business imperative for the planet. Changing workplace and workforce expectations Emery and Trist (1965) developed a typology describing four types of environment in which organizations operate and identified the appropriate organizational response. Environmental analysis with PESTLE demonstrates that the organizations 2030 will deal with high dynamism and high complexity pressures from outside. The organizational responses recommended by Emery and Trist (1965) for such environment types are developing fluid organization structures, flexible managers and staff at other levels. Duncan (1979) argues that what really matters is only the management perception. If management does not perceive the environment as turbulent the organizational response will not be as described. Anyway the reality out there has to be observed, studied, analyzed and reacted in an appropriate manner to stay successful (Huczynski and Buchanan, 2007). Being under pressure of future challenges which come from globalization, new era of technologies, social and demographic changes organizations will have to adapt their internal structures, processes, strategies and leader behaviors to enable them to deal effectively with external changes. Many factors that define the workplace 2030 have already become evident alerting the organizations that the time has come to start preparing today for the challenges of tomorrow. PriceWaterHouseCoopers (2010) forecasts three worlds and business models for the future (Figure) This picture of life in tomorrow combines the events and trends which will shape the future and PwC suggests all 3 worlds can co-exist in some form distinct by geographic principle or industry sector. Researchers expect the following key trends to shape the workplace and workforce 2030: Talent will be worldwide. Social networks and future technology will increase the capability for talent to network and for companies to access talent globally and 24/7. Facebook, LinkedIn, Twitter and other social networking sites will become ordinary recruiting sources. As social recruiting spreads from few trendsetters to all the rest companies worldwide it be essential for both employees and employers to develop new set of skills to be successful in this way (The 2020 workplace, J. Meister, K. Willyerd, 2010). But its also expected the War for Smart Talent (www.globalfuturist.com) to be continued and probably even become sharper. There is a serious mismatch between the types of individuals available for work and specific skills employers are looking for (Manpower Talent Shortage Survey 2009). Experienced candidates who possess the required skills will be heavily demanded by the organizations worldwide. Given the increasing collaboration and growing importance of teamwork companies will consider entire teams hiring as a competitive solution to the business problem (IBM CHRO Study 2010). Skills will be specialized. Cost reductions will be an essential part of business imperatives 2020 2030 leading to the dominance of knowledge -intensive work which will require an increasingly complex set of skills. Less generalist, more in-depth mastery of specialist skills will be required. PwC in the Future of work 2020 (2010) predicts that specialization will be highly prized in each of the 3 worlds (Figure ) and employees will do their best to develop the most sought after professional skills. By 2030 3 or even 4 generations will have to collaborate together in one business environment having different mindset, background, expectations and live values. Most of the Baby Boomers cohort representatives who are now holding many CxO positions will have retired, taking with them lots of knowledge, experience and valuable expertise. It will force the companies to elaborate on ways how to retain the knowledge within the organizations developing new employment structures for soon-to-be retiring people (L. Gratton 2011). Or what we call retirement now will no longer have a traditional meaning? In terms of their characteristics, lifestyles, and attitudes, Boomers were the most populous and influential generation of all. They have defined themselves by their careers and many are workaholics. Their work is seen as a form of self-fulfillment, status, and proving th emselves, i.e., job titles matter. They have crowded into cultural careers such as teaching, religion, journalism, marketing, and the arts. Team orientation is valued. They are anticipated to work longer and harder (Eisner, 2005; Koco, 2006). They are very concerned about health and energy, looking for the sources to stay young, strong and active. Generation X will be in their mid-60s by 2030. They grew up in the time of economic uncertainty under the pressure of political and economic instability. Many wage and job opportunities for young workers were limited until the economic boom of mid-1990s. (Eisner, 2005; Lager, 2006). Success has been less certain for this generation. They do not believe in positive future time, energy, and relationships for advancement like the Boomers did. They have an economic and psychological survivor mentality and consider hard work as necessity. They had to compete in a weak global economy. Diversity and thinking globally are core values of Gen Xers ( Cranston, 2008; Francese, 2004; Ritson, 2007). The core driving business force by 2025 2030 will be generation Y aged 35 45. They witnessed the fast technological evolution with the Internet, Google, eBay, cell phones, PDAs, GPS becoming an admirable part of their lives. Integral parts of their lives is globalization, global citizens, working beyond fiscal boundaries (Hawkins, et al., 2010; Cranston, 2008; Eisner, 2005; Dietz, 2003; Gerritsen, 2008). Most of Gen Y has a strong sense of autonomy and independence, quite adaptable and comfortable in different situations. With blogging, YouTube, MySpace, Facebook, and Twitter anyone can become a reporter, producer, or social advisor. Social networking is a key distinction for this generation (Dickey and Sullivan, 2007; Donnelly, 2008). Gen Y demonstrates an entrepreneurial outlook and is more work/life balance oriented and less committed to an employer than previous generations but demonstrates high commitment when dedicated to a capt uring idea. It has been observed the this generations vales life styles, career development and overseas travel and may be the most adaptable yet in terms of technological skills and values intrinsic aspects of work such as mentoring and training in order to remain marketable (Cennamo Gardner, 2008) They expect flexibility in work hours, dress code, relaxed work environment, recognition for smallest effort and constant feedback and value opportunity to downshift to reach a holistic self-understanding and satisfying meaning in life (IBM, 2011). By 2030 Gen Z will be taking an increasing role in society and business life. But what is known that they will grow up influenced by the factors described above in PESTLE. That is supposed to be a global and diverse generation. The technol ogical progress and next generation of social networking sites makes it possible to build online communities that are more like someones closest group of friends. (Labi, 2008b; Soltan, 2004). PwC research (2011) revealed that Gen Z will be similar to Gen Y in their expectations. But some features will be even more vivid manifested like desire for work/life balance, rapid career progression, less concern about working hours, high interest in new projects and less commitment to one employer. In effect, an entire generation is training itself to handle more complicated tasks (IBM, 2011). Influenced by expected demographical changes described above organizations in Europe, North America, Canada will face the decline of Caucasian workers and increase of non-Caucasian. It is projected to double from 18% to 17% (The 2020 Workplace, J. Meister, K. Willyerd, 2010) While people managers are trained to deal with many forms of diversity, they will have to be prepared to manage workforce with greater diversity in age, gender and ethnicity. The rapid expansion of the digital universe and advancement in communication technologies will lead to the new meaning of being at work and create new ways on how people communicate with each other. Today for a number of jobs it is no longer needed for people to be physically located in the office to successfully perform job responsibilities and by 2030 it is forecasted most of the employers where the business allows will completely relieve their employees from working 9 to 5 office hours (Hewitt Associates, 2008). People will be free to decide where and how to organize their workplace. But Hewitt also emphasize that this capability can bring some employees new level of work/life balance satisfaction and for others its fostering workaholic tendency leading to lousy employees: tired, depressed, mistake prone, resentful, and eventually burned out (C. HEV infection represents a major global public health issue especially in developing countries, where the global death rate is 1-15% and up to 30% in pregnant women (3). Every year there are an estimated 20 million hepatitis E infection (HEV) are registered over the world. it is known that chronic liver disease by hepatitis E develops in persons who are undergoing immunosuppression, including individuals infected with HIV(4). Recently, many studies have demonstrated cases of chronic HEV infection (characterized by detection of HEV RNA greater than or equal to 6 months in plasma) and cirrhosis in immunocompromised patients, including organ transplant recipients(5) patients with lymphoma and haematological malignancies (4,6 ) and in persons infected with the human immunodeficiency virus (HIV) (7). A number of studies have suggested that In persons infected with HIV may acquire HEV infection more often than individuals without HIV (8,9). The first verified case of chronic infection E in 2008 (10), which can to lead to liver damage and develops of hepatic fibrosis and even cirrhosis in immunosuppressed patients such as Patients with human immunodeficiency virus (HIV) infection and solid organ transplant recipients (11-14). The seroprevalence of anti- HEV IgG in HIV-positive cohorts ranges varies from one geographic location to another from 1.5% to 11.2% (19,20). Incidence of infections caused by HEV, defined by detecting HEV RNA in the serum, i s low, ranging from 0 to 1.3% (21-23). However, studies regarding co-infection of HIV and HEV and HEV associated chronic liver disease in HIV-infected patients are limited in IRAN. The present study aimed to determine the prevalence of HEV RNA in HIV infected patients in Iran a country with moderately high prevalence of both infections. Therefore, in order to better understand the clinical impact of HEV infection in these populations we have conducted a study determining HEV RNA in HIV-infected patients. In the present study, we determined whether HEV RNA were present in serum samples obtained from HIV-infected patients to investigate the prevalence of, and factors associated with, HEV infection in HIV-infected individuals. In addition, epidemiological, clinical and analytical factors were analyzed in order to identify potential risk factors associated with HEV seropositivity. The aim of this study was to define the degree of prevalence of HEV RNA in the group of HIV infected patients in Iran. For this study real-time RT-PCR assays targeting ..genes have been developed. We report 6 cases of hepatitis E infection in HIV-infected patients that   none of our patients received ribavirin treatment. Methods and materials:       Study population and samplecollection: A total of 80 Iranian patients with HIV-1 infection attending the Tehrans hospitals were enrolled in this study from February 2015 to April 2016. The exclusion criteria considered as patients who had been received anti-retroviral therapy. This study followed the principles of the Declaration of Helsinki and study has been approved by a local research ethics committee of the Iran University of Medical Sciences, Tehran, Iran. The participants were informed all aspects of the current study, and Informed consent was obtained from all of the participants prior to their enrollment for current study. About 5 ml of peripheral whole blood Sample from each participant was collected into a sterile EDTA-containing Vacutainer tube and plasma was separated from blood via centrifugation and frozen at -70 oC until analysis. HEV RNA Extraction: Efficient HEV RNA extraction using the High Pure Viral Nucleic Acid Kit generates purified HEV RNA was extracted from 200 ml of plasma (Roche Diagnostics, Germany). RNA pellets eluted with the provided elution buffer and stored at -70 °C until analysis. cDNA Synthesis: For detection HEV RNA, Reverse transcription polymerase chain reaction (rReal Time PCR) was was performed by using the first strand cDNA synthesis kit by Revert AidcDNA synthesis kit from RNA templates (Thermoscientific, USA). HIV-1 viral load quantification: Measurement of blood plasma HIV-1 RNA concentration performed by COBAS TaqMan 48 (Roche Diagnostics, Hacienda Drive Pleasanton, CA, USA) kit in the patients plasma samples (500 ÃŽ ¼l) and high pure extraction was used according to the manufactures recommendation. This method is a Real-Time PCR based on dual-labeled hybridization probe which targets the highly conserved region of HIV-1 gag gene. Limit of Detection of the COBAS TaqMan HIV-1 Test is 48 to 107 copies/mL. Hepatitis E Virus real time PCR assay: A Real Time PCR assay was developed for detection of HEV RNA. Among 150 participants, 95 (63.3 %) were male and 53 (35.3 %) were female. . There was no significant correlation between age and HEV s RNA positivity (p value= 0.18) (Table 3). By measuring the viral load, we could examine the relationship between HIV viral load and HEV infection. A sufficiently large sample size is also necessary to establish this finding. The main conclusion of our study is that HEV infection important to consider in the differential diagnosis of otherwise unexplained chronic hepatitis in Iranian HIV-1-infected patients. Furthermore, our study revealed that HEV infection has moderate prevalence in the HIV-infected population of Tehran. Due to the HEV infection that may cause rapidly progressing chronic hepatitis in immunosuppressed HIV co-infection patients, with development of cirrhosis in the short term, Screening for HEV in HIV-infected individuals presenting Symptoms of Hepatitis or with hepatic fibrosis of unknown origin is warranted.

The owner scared of his uninvited company. At first the house’s owner seems to fear the ghosts, but he eventually comes to respect the bond that they still share.
